Artrite Reumatóide

O comprometimento pulmonar na Artrite Reumatóide (AR) é tema complexo e com incertezas. As lesões são heterogêneas e padrões distintos de pneumonite intersticial se associam com prognóstico diverso. Nesse corte da tomografia de tórax de alta resolução (TCAR) observam-se opacidades com distribuição periférica e também peribroncovascular, com poucas imagens em vidro fosco; nódulos centrolobulares compatíveis com bronquiolite folicular e bronquiectasias mesmo sem fibrose. Há discretas áreas de faveolamento. Na Artrite Reumatóide o faveolamento é menos comum que na Fibrose Pulmonar idiopática (FPI). De acordo com as Diretrizes ATS/ERS/JRS/ALAT de 2011 esse padrão é inconsistente com Pneumonia Intersticial Usual (PIU/UIP) e também pode ser chamado como Pneumonia Intersticial Não-Específica (PINE). Esse padrão costuma ter um prognóstico de resposta ao tratamento melhor que o padrão PIU/UIP. Padrões radiológicos típicos de PIU/UIP indicam prognóstico ruim.

Pulmonary involvement in rheumatoid arthritis (RA) is a complex subject with uncertainties. The lesions are heterogeneous and distinct patterns of interstitial pneumonitis are associated with a diverse prognosis. In this high-resolution chest CT scan (HRCT), opacities with peripheral and also peribronchovascular distribution are observed, with few ground-glass opacities; centrollobular nodules compatible with follicular bronchiolitis and bronchiectasis even without fibrosis. There are discrete areas of honeycoombing. In rheumatoid arthritis, honeycombing is less common than idiopathic pulmonary fibrosis (IPF). According to the 2011 ATS / ERS / JRS / ALAT Guidelines, this pattern is inconsistent with Usual Interstitial Pneumonia (UIP) and may also be referred to as Non-Specific Interstitial Pneumonia (PINE). This pattern usually has a better prognosis of treatment response than the PIU / UIP pattern. Radiological patterns typical of UIP / UIP indicate poor prognosis.

Admite-se que a presença na TCAR de extensas áreas em vidro fosco ou consolidações indique maiores chances de resposta ao tratamento imunossupressor e melhor prognóstico. Nesse corte observa-se o padrão de BOOP (bronquiolite obliterante com pneumonia em organização), também chamado de pneumonia em organização criptogênica (POC-COP), presente em menos de 10% dos pacientes e cursa geralmente com melhor prognóstico. Nesse corte observa-se distorção da arquitetura pulmonar no terço médio à direita com consolidação e bronquiectasias. Bronquiectasias sem fibrose (seta vermelha), nódulos compatíveis com bronquiolite folicular (seta verde), vidro fosco (seta azul) e fibrose sub-pleural.

It is assumed that the presence of large areas of ground glass or consolidations indicates a greater chance of responding to immunosuppressive treatment and better prognosis. In this section, the pattern of BOOP (bronchiolitis obliterans with organized pneumonia), also called cryptogenic organization pneumonia (POC-COP), is present in less than 10% of the patients and usually presents with a better prognosis. In this section, there is distortion of the pulmonary architecture with consolidation and bronchiectasis. Bronchiectasis without fibrosis (red arrow), nodules compatible with follicular bronchiolitis (green arrow), frosted glass (blue arrow) and sub pleural fibrosis.

Nesse corte inferior dos pulmões as lesões em vidro fosco são discretas, periféricas, sub-pleurais, discreto faveolamento e predominam nas porções posteriores dos lobos inferiores.

In this lower CT slice of the lungs the ground glass lesions are discrete, peripheral, sub-pleural, discrete honeycombing and predominate in the posterior portions of the lower lobes.

Nesse corte inferior dos pulmões observa-se distribuição difusa das lesões nos lobos inferiores e nas regiões posteriores dos pulmões. Faveolamento periférico discreto com espessamento dos septos interlobulares (seta azul) e poucas opacidades em vidro fosco. A presença de bronquiectasias (setas vernelhas) na Artrite Reumatóide costuma ser sinal de mau prognóstico.

In this lower section of the lungs, diffuse distribution of the lesions is seen in the lower lobes and in the posterior regions of the lungs. Discrete peripheral furrowing, interlobular septal thickening (blue arrow) and few ground glass opacities. The presence of bronchiectasis (red arrows) in rheumatoid arthritis is usually a sign of poor prognosis.
